Production Risk Identification and Assessment of Prawn Crackers Product using Fuzzy Failure Mode and Effect Analysis: A Case Study

Abstract PT. XYZ is an agro-industrial company that produces and exports prawn crackers. During the production process, this faced various risks of failure such as non-fulfillment product standards. Therefore, proper risk identification assessment are needed. This study aims to identify analyze potential modes (FM) each stage cracker using Fuzzy Failure Mode Effect Analysis (Fuzzy-FMEA) method. Identification was carried out determine FM, causes, effects. Then, priorities based on value Risk Priority Number (FRPN) identified FM. The results showed there were 45 FM from 10 stages process. for raw materials not meeting organoleptic test at preparation stage, damage mixing machines equipment uncooked cuboid dough contamination forming steaming cooked pressing pre-drying chilling during slicing drying packaging final storing stage.
